2020-2021 Cruz Health Center Requirements
Dear Middlesex School Parents and Guardians:
In order for the Health Center to provide the best care to your student it is imperative that we have the most up-to-date information and permissions. All of this information is collected and stored through the Magnus Health Portal.
All requirements should be completed through the Magnus Health Portal by May 15th. We understand that you may have an appointment for a physical later in the summer. The forms that must be completed by the primary care provider can be uploaded when they are available but hopefully no later than registration and orientation. We understand that, based on the current COVID19 pandemic, appointments for physical examinations may not be available. Please contact the health center if you are not able to secure an appointment before the beginning of the school year.
Please use the same login information for MyBackpack that was sent to you to login to Magnus Health Portal. You will be guided to fill out the health questionnaire in the Vital Health Record as well as complete all of the other requirements. If you are an international family you will be required to purchase the health insurance policy offered through the school. Information to sign up is accessed through the Magnus Health Portal as well.

Please note that your child will not be allowed to participate in athletics or move into a dormitory until all requirements are completed. You will receive weekly reminders as long as there are outstanding requirements in your account.
